Pursuant to an arrangement with Dalton Associates, Greenspace Mental Health (the “Company”) has made available a platform that facilitates the measurement of patient progress and outcomes and displays such information to patients and their health care provider(s). The purpose of this Privacy Policy is to set out the manner in which the Company collects, uses, discloses and otherwise manages personal information. This Privacy Policy applies to the privacy practices at (“Website”) and offline collection points. Please see the Patient Privacy Policy for details regarding the Company’s personal information practices for registered patient users of the platform.

How We Collect and Use Your Personal Information

Below is a summary of the ways we collect and use your personal information:

Interacting With You: When you contact us with a comment, question or complaint, you may be asked for information that identifies you such as your name, address, email address and a telephone number along with any additional information we may need to promptly answer your question or respond to your comment or complaint. We use this information to respond to your inquiry and improve our service offerings. We may also use personal information to establish and manage our relationship with you and provide quality service.
